Check the lugs closely to see if it has been altered.

Sometimes it's not easy to tell.
Generally you'd want to make sure the lugs are "thick" and aren't thinner than they would be if new. You'll also want to look at the crown guards for similar signs of being smaller/thinner/sharper than if new. The beveled edge of the case can help identify a polished case...as well as the lug holes (if you have them). Overall, the proportions, shape, and flow of the case shouldn' look any different than if it was new. Often these signs are not obvious and take some practice to identify.

Lugs might look somewhat rounded.

Interesting topic, ive often wondered about this one... anyone got some good macro shots of polished and non polished they could post perhaps? That would be really helpful! :-)

It seems to me that the watches are polished at the factory when new (see the 904 Steel Video) so I can't imagine why a watch could not have light wear polished out without any visible differences.

It has been really hard me to figure out from the pictures and I have owned couple of rolex watches from last 10 years.
