In my opinion the insert is fake.
The triangle hand is not adequate for 1966.

my father in law has a '66 1675, and his is a small pointer GMT hand. perhaps this one has been replaced. the dial looks good, though. the lettering is correct. if it's a fake, it's a pretty good one. either that, or it's a "real" rolex cobbled together from a couple of later GMTs.

though i will say, the more i look at that GMT hand, the more it looks wrong. it's not very well made - you can see a cream color around its edges. and generally, if something on a rolex looks cheap or poorly fabricated, it's fake.

The red hand has obviously been replaced. The rest of the watch appears original. Having the 2nd hour hand replaced, or any of the hands replaced, is not uncommon. Personally, I'd have replaced the other hands if it were mine for a matched set.
