Rolex Knickknack Questions

A few questions:

1) When did Rolex start and then stop giving out the polishing cloth with the Rolex/Rolex Cellini logos?

2) When did Rolex stop giving out the anchor with Submariners?

3) I have a near complete set for an S Serial (~ 1993) Sub Date - the original warranty certificate is missing. My "Rolex Submariner" booklet is from 1.1992 and my "Your Rolex Oyster" booklet is from 7.1993.

Was/is it normal for ADs to randomly put together whatever booklets they have in storage when putting together kits at point of sale? I was surprised to see the dates not being in line.
